Registration Forms
These copies can be filled out and printed. Please turn into the committee chair. An electronic youth registration form can be found at the bottom of New Parent's Corner.
Health Forms
All scouts, youth and adults, should have an annual health and medical record on file with the troop to attend scouting events.
Scouting Events
For day and weekend trips you will need to fill out parts A, B1 & B2 of the Medical Form. This form will need to be updated annually.
Summer Camp
For summer camp and other extended trips part C of the Medical Form will need to be filled out by a medical professional. A physical must have been completed within 12 months of going on the extended trip.
High Adventure Trips
For high adventure trips you will need to add part D of the Medical Form as well as the Risk Advisory. Forms are specific to the adventure base.
